Cardiac Care for your Loved One
WHAT WE DO
We are pleased to announce our Heart 2 Heart cardiac care program at Center for Hospice Care. This program allows our nurses the opportunity to care for your loved one and manage cardiac symptoms in the home, rather than in a hospital or emergency room.
when it comes to managing a cardiac diagnosis, consider Center for Hospice Care: your only community-based nonprofi t agency dedicated to caring for your loved one with a life-limiting diagnosis.
Care is focused on controlling symptoms such as pain, shortness of breath, nausea, fatigue, swelling, weight gain, anxiety, depression, and uncomfortable symptoms in the home. when it comes to patient care! We collaborate with cardiac physicians, our agency medical director, social workers, and pharmacists to meet the care needs of you and your family. Our nurses have received training in the management of cardiac symptoms using evidence-based practice resources to provide the most up-to-date care in the home setting.
We offer an interdisciplinary approach to patient care. Members of our health care team include:

HOW WE MANAGE CARDIAC SYMPTOMS
Nurses at Center for Hospice Care received specialized cardiac training to control symptoms in the home.
We provide patient-specific teaching for diet and fluid intake as well as weight management for fluid retention. In-home scales will be provided at no cost to patients receiving hospice care (if needed). We provide in-home cardiac medications, “comfort kits,” to our hospice patients. The medications we use are the same as the ones used in Emergency Rooms: Lasix, Nitroglycerine, Oxygen, and Morphine.
A primary care nurse will be assigned. This nurse will be responsible for coordination of care through Center for Hospice Care‘s Interdisciplinary Group and your physician.
In the event of an emergency, our nursing staff is available 24 hours per day, seven days per week, and on holidays.
We offer home health aides as part of our program to assist with personal care needs & volunteer services to provide respite, complementary therapy, and friendly visitors.
Social Work/Chaplain services are available to assist with counseling/spiritual support and community resource support.
