Bereavement Coordinator - Part time

Center for Hospice Care is seeking an experienced and compassionate individual to be the Bereavement Coordinator. This dynamic individual will report to the Supervisor –Counseling Services and will work in collaboration with Nurse, Social workers and chaplains. S/He will be responsible for developing, delivering and coordinating the activities of Center for Hospice Care’s bereavement program.

Snapshot of key accountabilities:

  • Participates in weekly IDG meetings - Begins assessment of potential bereaved individuals and families upon admission and during course of care.  Uses appropriate medical/social history, interview data and other relevant diagnostic information to obtain an accurate assessment of patient and family needs in relation to issues of dying and death.  Documents in patients’ charts according to agency policies and procedures.
  • Initiates and implements bereavement follow-up for individuals and families to optimally meet their needs.
  • Documents in bereaved’s file according to agency policies and procedures.
  • Assesses, evaluates and documents patient / family grief needs and risk factors associated with grief, and responds as appropriate.
  • Develops and modifies bereavement plan of care as needed.
  • Provides individual and family counseling, extends to residents of a SNF/ALF or ICF/MR when appropriate and identified in the bereavement plan of care.
  • Provides for facilitation of community support groups.
  • Assesses bereaved family needs for advocacy, information and referral to other services.
  • Remains available to families during bereavement period.
  • Oversees the processes for calls and cards to be made/sent by bereavement volunteers.
  • Assists with the Annual Memorial Service in conjunction with the Community Development department.
  • Tracks, compiles and utilizes Bereavement statistics.
  • Develops and maintains list of community resources.
  • Provides educational services to Hospice staff and members of the community.
  • Enhances own professional growth and self-development.
  • Actively supports the organization’s mission, goals and performance improvement.
  • Follows organizational policies, procedures and standards.
  • Attends and participates in required meetings.
  • Able to relate to dying patients, families and IDG members during periods of grief and morning.

Minimum Qualifications

  • Bachelor’s Degree or equivalent with relevant work experience in grief/loss support. 
  • Demonstrated experience with intake and assessment process, crisis management and community resources. 
  • Clinical experience in the area of grief and loss, family therapy and group activities.
  • Experience in developing groups, program development, or management.
  • Ability to work as part of a team as well as function independently and coordinate activities as appropriate to accurately complete tasks in a timely manner.
  • Understands and maintains confidentiality.
  • Complies with HIPAA regulations.
  • Ability to relate well with staff, general public or individuals with a life limiting   illness and their families during a stressful period.
