Employee Benefit Eligibility
Full-time and part-time employees of Center for Hospice Care working 30 hours per week or more, unless otherwise noted, are eligible for the following benefits.
Employees receive 10 paid holidays, per the employee’s scheduled hours, including: New Year’s, Martin Luther King, President’s, Memorial, Independence, Labor, Columbus, Veteran’s, Thanksgiving, and Christmas.
Paid Time Off
An employee working at least 22.5 hours per week is eligible for PTO (Paid Time Off for vacation and personal time) benefits. PTO is accrued at 60%, 80% and 100% and is based on the employee’s scheduled hours. In addition, sick time will also be accrued.
Other Center for Hospice Care Benefits
Mileage reimbursement, employee referral bonus, longevity bonus, professional training courses/seminars, credit union availability, and various employee engagement, communication and recognition events.
Employee Assistance Program (EAP)
Every employee, regardless of hours worked, may access EAP services at any time at no expense for a specified number of appointments. This valuable benefit is designed to provide free, confidential, short-term counseling and referral by professionals trained to deal with a wide range of personal and workplace wellbeing concerns. The Center for Hospice Care EAP is provided through Solutions EAP.
Employees working 30 or more hours receive medical insurance coverage for themselves, their spouse or domestic partner and their dependents, if they so choose. Eligibility begins the 1st of the month following hire. Costs are shared by the Center for Hospice Care and the employee.
This is an employee-paid benefit for employees working 30 or more hours. Eligible employees elect dental insurance coverage for themselves, their spouse or domestic partner, and their dependents. The Center for Hospice Care Dental Plan is offered through Delta Dental. Eligibility begins the 1st of the month following hire.
Each employee, if hired to work at least 20 hours per week, is eligible to make salary deferral contributions immediately through payroll deduction. Employee contributions are 100% vested. The Center for Hospice Care 403(B) is administered by the Principal Financial Group.
Other Voluntary Benefits
Employee-paid benefits are funded through Payroll Deduction, if opted by employee. Insurance coverage includes: Life, Accidental Death & Disability, Short-term Disability, Long-term Disability, Long-term care, Cancer. Employee may purchase voluntary benefits for self, spouse and/or children.
