A community resource, ready to serve
Hospice is too often thought to be strictly for those facing fatal illnesses. It is much more than that. Center for Hospice Care is a non profit agency whose mission is to serve patients, families, and caregivers throughout the community – and we have many ways to help. Here’s an overview.
Hospice at home: The most significant service we provide is caring for patients in their own home, using an interdisciplinary group model to coordinate services for the patient and the family.
Hospice at large: When necessary and appropriate, we provide palliative and hospice care in skilled nursing facilities and hospitals.
Volunteers everywhere: With more than 140 volunteers, we are able to extend the reach of our care and service to many people in New London County.
We Honor Veterans: We provide special services for veterans, and consider it an honor to do so.
End of Life Planning: We have resources for patients and families who need to begin planning for the end of life. Examples: Living Wills, Advance Care Planning, burial planning, and more.
Bereavement Services: For anyone coping with grief and loss, we offer programs run by professional counselors and Center for Hospice Care staff to ease the path forward to emotional recovery and well being.
Public Education: Center for Hospice Care has programs for the public as well as professional organizations that cover a wide range of topics that are ideal for speaking events, presentations, and fundraisers.
Special Programs: In addition to our core services, we have developed several unique programs to meet the needs of specific groups, such as people who live alone and those with advanced dementia.
Event Sponsorship: We are always seeking ways to extend our message and presence into the community, and frequently host and co-host public and private events.
