
Skilled Nursing Facilities
In January 2005 Hospice Southeastern Connecticut embarked on a Skilled Nursing Facility initiative “Partners in Palliative Care” to better serve hospice appropriate patients in Skilled Nursing Facilities. A dedicated team of clinicians was identified to work exclusively with the Skilled Nursing Facilities. A partnership between Hospice Southeastern Connecticut, experts in end of life care, and the Skilled Nursing Facilities, experts in long term care, promoted optimal end of life care for facility residents and their families.
Hospice Southeastern Connecticut partners with more than 14 area Skilled Nursing Facilities as well as many Assisted Living Facilities in Southeastern Connecticut to allow the residents access to hospice care.
Together Hospice Southeastern Connecticut and the facility collaborate to ensure:
- Optimal Pain and Symptom Control
- Access to Volunteers for Companionship
- Access to Complimentary Therapies Including:
- Massage Therapy
- Reiki
- Aromatherapy
- Healing Touch
- Reflexology
- Psychological Support for Patient and Family
- Spiritual Support for Patient and Family
- Participation at Care Plan Meetings
- Bereavement Support for Family
- Support Groups for Facility Staff
