
Palliative Care Program of Southeastern Connecticut
The diagnosis of any progressive life limiting illness introduces a host of physical, emotional and social changes that impact the individual and the family. Bringing together the skills and knowledge of our inter-disciplinary team, the Palliative Care Program of Southeastern Connecticut addresses the issues, problems and concerns that patients and families will confront.
Palliative care is defined as “serving to relieve or alleviate without curing.” The Hospice Southeastern Connecticut Palliative Care Program allows patients to continue treatment while discussing their options, choices, goals for care, at the same time managing difficult symptoms. The Palliative Care team works with the patient’s physician and our Interdisciplinary Group whose members have expertise in symptom control and pain management and may provide a continuum of uninterrupted quality care throughout the course of the illness.
Who is appropriate for the Palliative Care Program?
The Palliative Care Program accepts patients who:
- Have a progressive life threatening illness that is no longer curable
- May continue to seek treatment
- Meet the criteria for home care payment through their personal medical insurance
- Emphasizes pain and symptom management, and enhances patients’ abilities to tolerate medical treatment
- Provides emotional support and guidance during difficult and complex treatment choices
- Facilitates clear and concise communication with patient, family and medical community as goals and options change
- Assists patients and families as they navigate the healthcare system
- Assists with Advanced Directives and end-of-life planning when appropriate
