Our Caregiving Team

All of the Hospice Southeastern Connecticut team members receive special training in the care of terminally ill patients. We are skilled at keeping patients physically comfortable and understand the psychosocial impact of caring for a dying family member at home. We develop a plan of care to maintain the patient at home or in the place where they live or in alternate facilities if home care is not possible. We provide equipment and medications, help with household chores and transportation, aid with bathing and grooming are just some of the ways we ease the process.

Our Interdisciplinary Team includes:

  • Medical Directors (hospice physicians)
  • The Patient's Personal Physician
  • Hospice and Palliative Care Certified Nurses
  • Medical Social Workers
  • Hospice & Palliative Care Certified Home Health Aides
  • Physical, Occupational and Speech Therapists
  • Registered Dieticians
  • Pharmacists
  • Multi-denomination Clergy
  • Trained Volunteers
  • Bereavement Counselors
